Waiting List Application

The application can only be completed if your child(ren) is enrolled at Fall Creek Elementary School in grades Kindergarten through 5th grade. . 

To be added on the Waiting List please submit the application below and email fallcreeksap@gmail.com and fallcreeksap.asst@gmail.com to verify you have successfully been added and to receive a placement number. 

If your child(ren) is at the top of the Waiting List and a space becomes available during the school year, you will be contacted through email with further instruction.
